Form G-28: Notice of Entry of Appearance as Attorney or Accredited Representative

The purpose of this form is to provide notice that an attorney or accredited representative of a religious, charitable, social service or similar organization will appear before U.S. Citizenship and Immigration Services on behalf of a person involved in a matter before USCIS.


Form I-9:  Employment Eligibility Verification 

All U.S. employers must complete and retain a Form I-9 for each individual they hire for employment in the United States. This includes citizens and noncitizens. On the form, the employer must examine the employment eligibility and identity document(s) an employee presents to determine whether the document(s) reasonably appear to be genuine and relate to the individual and record the document information on the Form I-9. The list of acceptable documents can be found on the last page of the form.


Form I-193:  Application for Waiver for Passport and/or Visa 

This form is for an alien who is a legal resident of the United States to reenter the United States without the alien’s passport and/or visa.


 Form I-130, Petition for Alien Relative

This form is used by U.S. citizens and permanent residents to bring qualifying family members to the United States.


Form I-539 Application to Extend/Change Nonimmigrant Status

This form can be used to extend status in certain immigrant classifications or to change from one status to another.  This form is commonly utilized by applicants for permanent residency who are currently residing in the U.S. in a different immigration status.


Form I-730, Refugee/Asylee Relative Petition

This form can be used by refugees and asylees to bring qualifying family members to the United States.


Form I-864, Affidavit of Financial Sponsorship

Some cases require this form which names a financial sponsor for an intending immigrant and demonstrates that the immigrant will not become a public charge.  Completion of this form is a requirement in most family based petitions.
